Over the last 20 to 30 years the history of non-invasive cosmetic procedures has been very tumultuous. There have been a number of technologies that have disappointed both physicians and patients in delivering the results they have promoted. A conservative and very critical physician will approach new technology with skepticism until proven otherwise. As to where the fat goes, there are two main pathways. The first is your body could utilize or “burn” the stored energy of the fat during your daily activities or exercise. Redness and hotness are normal in the short-term following CoolSculpting.  It is also normal to experience transient numbness, bruising, soreness, and mild warmth in the treated area. It is however unusual to experience redness and hotness at the 2-week mark. I have seen patients with a post-treatment course similar to yours as a result of mild panniculitis (an inflammatory process of the fat layer) following CoolSculpting. I recommend a follow-up with the treating physician for an accurate diagnosis.

The instances of paradoxical adipose hyperplasia (PAH) is very rare. The new Advantage applicators have reduced the instances of PAH even further. At this point, the etiology of PAH is unknown and there is currently no evidence that diet and/or exercise would influence the instances of PAH. Diet and exercise will however complement your CoolSculpting results and offer the best possible overall outcome.

QUESTION

Hello – I went to Ideal Image for a consultation for CoolSculpting. I’m 5’5 and 145 pounds. To do upper and lower abdomen she gave me a quote with 8 applicators – I’m assuming she quoted me with the smaller applicators – each being 35 minutes and all in one sitting. I haven’t seen any reviews with this many applicators – does this seem right?

DR. GERRISH ANSWER

Unfortunately, without an in-person examination, it is very difficult to assess the number of cycles you would need to completely treat your upper and lower abdomen. However, I can say that multiple cycles in one appointment are completely safe. For the best results, it is recommended to follow the Treatment to Transformation protocol. The Treatment to Transformation protocol dictates that the entire cosmetic area (i.e. stomach, love handles, back, etc) be treated fully to obtain the best body contouring response. Blood work looking at cholesterol, liver function, triglycerides, etc were analyzed and found to be stable in the studies used to obtain CoolSculpting’s FDA clearance.

QUESTION

How do I know if my therapist is qualified in CoolSculpting?

ANSWER BY DR. GERRISH

QUESTION

CoolSculpting on inner and outer thighs. One side feels normal, the other side is tender. Does that mean it didn’t work?

My outer right thigh is swollen and much more tender yet numb than any other part of my thighs. I have slight tenderness on the left inner thigh. The treatments were done to the right inner and left outer and then switched to the opposite sides 5 days ago. Is it possible only one side worked? Is it possible that the tender swollen side is Paradoxical hyperplasia that people speak of? I’m a little concerned. I’ve been massaging the area and drinking tons of water as directed. Please advise.

 

ANSWER BY DR. GERRISH

It is very common to experience varying degrees of numbness and tenderness in CoolSculpted areas. The good news is that there is no correlation between post-treatment tenderness and the effectiveness in fat reduction. Paradoxical Hyperplasia (PAH) is a very rare occurrence (1 in 25,000 with the new Advantage applicators) and is typically not seen until about 4 months after treatment. Additionally, there appears to be no correlation between the amount of bruising, tenderness, or numbness and the development of PAH. Massaging the area is an important aspect of the treatment immediately after removing the applicator. Continued massage days after the treatment will not offer additional benefit and may actually increase the tenderness you are experiencing.

QUESTION

I measure every week with no change today is my 6th week —at least the soreness has lessened some–still same “belly”-due for the second procedure on August 16th if I can handle pain–what should I get for after –rub on cream???? should I have it done again on 16th-past 6 weeks have been awful–HELP

 

ANSWER BY DR. GERRISH

With CoolSculpting you may begin to see an initial response around the 6th week. On average, it will take 12 to 16 weeks to see the final result. With you being at 6 weeks, I would not expect to see any significant changes yet. You still have another 6 to 10 weeks of improvement. While soreness can be present several weeks after the treatment, it is unusual to persist into the 6th week. I would recommend you follow up with your CoolSculpting provider to discuss your ongoing discomfort and to possibly push your second treatment further into the future until the area has fully recovered

QUESTION

I went for a Coolsculpting consult for my saddlebags at a new medical spa. The doc said the procedure will take two hours long: one hour for each side. I saw the applicator and it is not the one that uses suction. From what I’ve been reading, the process should take 2 hours on each side=4 hours total. Can someone clarify, please?

 

ANSWER FROM DR. GERRISH

The outer thigh or saddlebag area can not be treated with the traditional suction applicators. There are two generations of applicators currently available to physicians that can treat the outer thigh, the CoolSmooth, and the CoolSmooth Pro. The CoolSmooth has a treatment time of 2 hours (4 hours in total) and the CoolSmooth Pro is 75 minutes (2 1/2 hours in total). Both applicators will have similar efficacy. Keep in mind that practice with two machines may be able to halve the treatment time by DualSculpting (using both machines at the same time).

QUESTION

Would it be helpful to wear a compression hose following to alleviate swelling?

ANSWER FROM DR. GERRISH

The post-treatment course has changed significantly since the release of the new Cool Advantage applicators. The amount of swelling, bruising, and post-treatment discomfort has been dramatically reduced. From a medical standpoint, compression following CoolSculpting is not necessary. With that being said, some patients have reported that compression garments do help with the soreness they may experience during the first week after CoolSculpting. I always advise patients to try compression if they do experience some post-treatment soreness and assess if it offers some relief.

QUESTION

So I was curious I have the basics of where CoolSculpting is done on the lower body. However is it possible to do the front sides of the legs and beside the knees? I wanted to have slimming done on my lower half however I’d prefer not to go invasive if possible.

ANSWER BY DR. GERRISH

Over the last several years CoolSculpting has expanded its FDA clearances to include areas on the legs, specifically the inner thigh, the outer thigh, the ‘banana roll’ – the bulge just below the buttocks, and above the knee. For all of these areas, a careful evaluation must be performed to assure the bulge is amenable to treatment and the applicator geometry. To date, there is not an applicator for treating the front side of the leg other than just above the knee.

QUESTION

I had coolsculpting on my stomach but the technician only very briefly rubbed the area. I have read that massaging is very important to the coolsculpting results. Will I still see a good difference in the treated area even without the massage?

ANSWER BY DR. GERRISH

The most up-to-date protocol we teach at CSU is to perform a 2-minute manual massage after removing the treatment applicator. CoolSculpting will cause fat reduction with or without the massage, however, it has been shown that the manual massage will further increase the fat cell destruction, i.e. results when done properly. An experienced CoolSculpting provider may limit the massage or forgo it completely if the goal is to ‘feather in’ a bulge that purposefully needs less fat reduction. A discussion with the treating practitioner may help you in understanding your treatment

QUESTION

How cold do you have to freeze the fat with a cool Machine for it to really freeze those cells like -10 below? And will it ” burn” the skin being that cold?

ANSWER BY DR. GERRISH

The temperatures used in Coolsculpting have been rigorously evaluated to maximize fat reduction while maintaining a substantial safety margin. Continual temperature monitoring is utilized during the treatment to assure safety. During the treatment, the skin reaches about -11 degrees Celsius with the superficial fat getting to about -8 or -9 degrees Celsius and the deeper fat reaching approximately 0 to +4 degrees Celsius. The temperature of the fat is the critical aspect of CoolSculpting to achieve cryolipolysis or fat cell destruction. The special CoolSculpting wipes used on the treatment area lowers the freezing point of the skin and allows the skin to withstand the colder temperatures

QUESTION

I had CoolSculpting done to my outer thighs yesterday. By the time I got back home, I was hardly sore and now there is only a dull ache. No itching, tingling, numbness, and not that much or no swelling (non that I can notice). Did I just get lucky or is my body too resistant to the treatment/ the treatment isn’t working?  I went to a very professional well-reviewed and certified office to get this done.

The applicator used for treating the outer thighs has a drastically different post-treatment course than the traditional CoolSculpting suction applicators. In general, the outer thighs have less swelling and rarely bruise. Itching and tingling, if you are to experience it, usually happens several days after the treatment. You should expect to see the beginning of your results in 4 to 8 weeks.

QUESTION

Does Coolsculpting get rid of or change the number of fat cells on your body?

ANSWER BY DR. GERRISH

As adults, the total number of fat cells is fixed and we are not able to create new fat cells unless we become morbidly obese. With CoolSculpting the treated fats cells are destroyed permanently. Since CoolSculpting has reduced the number of fat cells and we are not able to create new ones, the total number of fat cells in the body will be permanently reduced.

QUESTION

CoolSculpting: Would getting rid of extra water weight before a session help freeze more fat cells?

ANSWER BY DR. GERRISH

Altering your body’s water balance prior to CoolSculpting will not influence your final results. The cooling profile from the treatment is delivered and monitored accurately so each patient gets safe and consistent results.

QUESTION

may I know Zimmer Cryolipolysis is a CoolSculpting treatment but just using a different machine? how was the result for Zimmer? Does any surgeon recommend Zimmer compare with Zeltiq? because the prices are very different and my biggest concern is the EFFICIENCY and result. I plan to go for lower abdomen treatment. please advise me as I’m a bit confused now.

The Zimmer Corporation manufactures a medical-grade cooling device that assists in both skin cooling and pain control for dermatological procedures. The Zimmer does not have the ability to create any changes and/or reductions in the fat layer. CoolSculpting on the other hand uses cryolipolysis to cause fat cell death resulting in non-invasive body contouring. In regards to the most efficient and successful non-invasive body contouring treatment, it is my feeling that CoolSculpting is the current gold standard.

QUESTION

I had coolsculpting 4 weeks ago nearly on my upper and lower stomach but I still haven’t seen results? Also, I want to get in on the roll of fat just under my bra line but would this be classed as upper stomach?

ANSWER BY DR. GERRISH

It is absolutely normal and to be expected that at the 4-week mark you do not see much or any results from the CoolSculpting treatment. On average, patients will see the start of the results at 6 weeks. By 8 weeks patients typically see 50% of the results and by 12 to 16 weeks the full response is obtained.

While we all would love a one and done, CoolTones does require a series of treatments. For optimal results, most people require six treatments, within a three-month time frame. After that, maintenance is recommended every three to four months. The number of treatments you need to meet your goals will depend upon your current condition, medical history, and lifestyle. During your consultation, we will work with you to develop a treatment plan to provide you with the results you desire.

The cost for laser hair removal treatments is highly individualized and is dependent on the area being treated. When compared to various other hair removal methods, laser hair removal is an extremely cost-effective alternative. While many clinics play with low-ball price quotes to get you in the door, we don’t do that. Durning your consultion you will be presented with a very clear treatment plan and pricing schedule.

A Gerrish MedEsthetics Laser Specialist will assess the area and explain the treatment to you in detail. Treatment times vary but can be as short as several minutes. Immediately following the laser procedure your skin may have minimal reddening that will resolve over the next several hours. Each patient will be individually advised if specific post-treatment skincare is necessary.

With each laser pulse, the patient experiences a mild heat sensation on the skin’s surface most commonly compared to the snap of a rubber band. In sensitive areas, anesthetic creams or cooling devices can be utilized to minimize the discomfort.

For as long as I can remember, I have had to worry about extreme perspiration on a daily basis. Social situations are especially stressful, and I only wear certain colors and styles of tops due to my hyperhidrosis. Today I saw Dr. Gerrish for miraDry, and it was an excellent experience. Nordlys Ellipse IPL

The number of treatments depends on the location and the life cycle of your hair. Because every hair grows for a period of time, you may need to repeat the treatment at intervals until all hair follicles have been treated while growing.

Clinical studies have shown that 4 to 6 treatments are needed, and typically the intervals between treatments are 1 to 3 months.

The Nordlys™ system permanently reduces unwanted hair by delivering short, bursts of light directed at the target site. To ensure good contact with the skin, a thin layer of gel is applied to the skin prior to treatment.

Nordlys Ellipse IPL For Acne

Acne occurs when your sebaceous glands overproduce an oily substance called sebum. When extra sebum gets trapped in your hair follicles, your skin can become irritated — leading to the appearance of pimples on your face. Left untreated, some types of acne can cause permanent scarring.

The Nordlys™ system treats acne by reducing the blood supply to the sebaceous gland to slow down the production of sebum. The system delivers short, bursts of Ellipse IPL™ directed at the skin. Tell your health care provider if you are taking blood thinners or medicines that can interfere with the clotting of blood, such as aspirin or warfarin. These medicines might make it more likely that you will experience bruising or bleeding at the injection site. Tell your health care provider if you have any diseases, injuries, or disabilities of the hand if you have a history forming large, raised scars, or if you have had any other skin treatments such as skin peels.

One of the risks of using these products is unintentional injection into a blood vessel. The chances of this happening are very small, but if it does happen, the complications can be serious and may be permanent. These complications, which have been reported for facial injections, can include vision abnormalities, blindness, stroke, temporary scabs, or permanent scarring of the skin. If you have changes in your vision, signs of a stroke (including sudden difficulty speaking, numbness or weakness in your face, arms, or legs, difficulty walking, face drooping, severe headache, dizziness, or confusion), a white appearance of the skin, or unusual pain during or shortly after treatment, you should notify your health care practitioner immediately.

As with all procedures that involve an injection through the skin, there is a risk of infection.

Do not use RADIESSE® or RADIESSE® (+) if you have a skin infection until it has healed.

It is not known if RADIESSE® or RADIESSE® (+) is safe or effective in the lips, or in the area around the eyes.

The microspheres in RADIESSE® and RADIESSE® (+) can be seen in X-rays and CT Scans. It is very important that you tell your health care provider that you have had RADIESSE® or RADIESSE® (+) dermal filler.

If you have a history of herpes, you may experience a herpes breakout after receiving RADIESSE® or RADIESSE® (+).

Injection in the back of the hand may result in temporary difficulty performing activities. RADIESSE® may cause nodules, bumps or lumps in the back of the hand and can last up to 1 year.

You should minimize strenuous activity and avoid extensive sun or heat exposure for about 24 hours after treatment and until any swelling or redness has resolved.

RADIESSE® and RADIESSE® (+) are dermal fillers that are used for smoothing moderate to severe facial wrinkles and folds, such as nasolabial folds (the creases that extend from the corner of your nose to the corner of your mouth). RADIESSE® is also used for correcting volume loss in the back of the hands.

Restylane

The Restylane family of products should not be used by people who:

  • Are pregnant, breastfeeding, or under 22 years of age
  • Have recently used drugs that thin the blood or prevent clotting
  • Have had previous bad allergies, particularly to microorganisms known as gram-positive bacteria
  • Have serious allergies to drugs that have previously required in-hospital treatment
  • Have bleeding disorders
  • Have a known allergy to lidocaine, if using Restylane Silk, Restylane Lyft, Restylane, Restylane Refyne or Restylane Defyne

Initial Restylane treatments are usually complete in one or two sessions, as needed. Depending on your individual body chemistry, you will work with your specialist to determine a maintenance schedule.

After your treatment, you may experience side effects, such as swelling, redness, pain, bruising, headache, tenderness, lump formation, itching at the injection site, and impaired hand function. These are typically mild in severity and normally last less than 7 days in nasolabial folds and less than 14-18 days in lips. Patients should be limited to 6.0 mL per treatment of moderate to severe facial wrinkles and folds. For the treatment of volume deficit in the hands, patients should be limited to 3.0 mL per hand per treatment. Swelling may be more likely in patients under 36 years, and bruising may be more likely in patients over 35 years. Depending on your volume needs, most Restylane treatment appointments last less than an hour and begin to work almost immediately by adding volume to smooth away wrinkles. A clear gel formulation of hyaluronic acid, Restylane products act like your body’s own hyaluronic acid (and eventually break down naturally). Each Restylane product is designed for a specific purpose.

Restylane Silk: In clinical studies, the results of Restylane Silk lasted approximately 6 months following treatment. In a U.S. clinical trial involving 221 mostly female subjects, 98% of subjects reported a visible improvement in lip fullness in the upper and lower lip (combined) 14 days after injection, and 77% still had lip improvement at 6 months following injection.11

Restylane Lyft: In a clinical trial involving 150 subjects, blinded evaluators observed that 90% of subjects treated with Restylane Lyft showed an

improvement in fullness in the right and left midface areas (combined) at 2 months, and 54% of subjects showed an improvement at 12 months. Additionally, 95% of subjects reported that they were satisfied with the appearance of their midface at 2 months, and 73% of subjects were satisfied at 12 months.3 In a clinical trial involving 89 treated subjects, blinded evaluators observed that 86% of subjects* treated with Restylane Lyft showed an improvement in the volume of the back of the hands at 3 months, and 76% of subjects* showed an improvement at 6 months. Improvement was defined as subjects with a Merz Hand Grading Scale (MHGS) score of ≥1 grade from baseline by the blinded evaluator’s live assessment.3

Restylane: In a clinical study treating the lines from the nose to the corners of the mouth (laugh lines), blinded evaluators reported that 95% of subjects improved for up to 18 months when they received an initial Restylane treatment, plus a follow-up treatment approximately 4.5 or 9 months later. Using this Restylaneregimen of treatments, blinded evaluators reported similar subject improvement, on average, at 18 months compared to their Week 2 Wrinkle Severity Rating Scale (WSRS) score.10

Restylane Refyne & Restylane Defyne: In a clinical study of subjects treated for laugh lines with Restylane Refyne and Restylane Defyne, investigators reported improvement in the appearance of wrinkles for up to 12 months in the majority of patients. Using the Wrinkle Severity Rating Scale (WSRS), a validated 5-point measure of the size and depth of the wrinkles, with grade 1 defined as the absence of wrinkles and grade 5 as extremely deep and long wrinkles, investigators reported that 79% of Restylane Refyne subjects and 77% of Restylane Defyne subjects had at least a 1-grade improvement on the WSRS after 6 weeks. Subjects also performed self-assessments (SSA) of wrinkle severity, with most reporting at least a 1-grade improvement in SSA scores with Restylane Refyne and with Restylane Defyne after 6 weeks.12,14,15

Dermal fillers are facial injection treatments used to help diminish wrinkles while restoring volume and fullness in the face and hands. There are several types of dermal fillers, one being hyaluronic acid. All Restylane products are transparent hyaluronic acids, which are sugars naturally found in the skin that help provide hydration, fullness, and elasticity.

The Restylane family of products is a brand of hyaluronic acid dermal fillers, which includes Restylane Silk, Restylane Lyft, Restylane, Restylane Refyne and Restylane Defyne. Each is a clear gel formulation of hyaluronic acid specifically formulated to act like your body’s own naturally produced hyaluronic acid. Each Restylaneproduct is designed for a specific purpose.1

Semaglutide

Semaglutide is not recommended for people with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2. It should also be avoided during pregnancy or breastfeeding. People with a history of pancreatitis should use semaglutide with caution

Some common side effects of semaglutide include nausea, vomiting, diarrhea, constipation, and abdominal pain. These side effects usually go away on their own or with dose adjustments. There is also a risk of more serious side effects such as pancreatitis and thyroid cancer, although these are rare.

Clinical studies have shown that semaglutide can lead to significant weight loss in people with obesity or overweight. In a 68-week study, participants who took semaglutide lost an average of 15% of their body weight, compared to 2.4% for those who took a placebo.

Semaglutide works by mimicking the effects of a hormone called GLP-1 in the body. This hormone regulates appetite and food intake. By activating the GLP-1 receptors in the brain, semaglutide can reduce hunger and promote feelings of fullness, leading to a reduced calorie intake and weight loss

Semaglutide is a medication that belongs to a class of drugs called GLP-1 receptor agonists. It is used to treat type 2 diabetes and has recently been approved by the FDA for weight management in adults with obesity or overweight with at least one weight-related health condition.

Threadlift

The final results of the PDO thread lift will vary from patient to patient but will most commonly appear within one to three months.
